At Pathways Senior Housing Consulting, LLC, our goal is to guide individuals and families through the process of identifying appropriate senior housing and care options with compassion, transparency, and integrity. This document provides important information regarding the services we provide and how our business is compensated. Nature of Services - Pathways Senior Housing Consulting provides senior housing advisory and placement services. These services may include: • Consultation regarding care needs and living preferences • Education about available senior housing options • Referrals to licensed care providers • Assistance coordinating tours and communication with facilities • Guidance during hospital or rehabilitation discharge planning • Support navigating the transition to a new living environment. Pathways Senior Housing Consulting does not provide medical care, legal advice, or financial planning services. Compensation Disclosure - Pathways Senior Housing Consulting may receive compensation from certain senior housing providers if a client chooses to move into a community that has a referral relationship with our company. This compensation is typically paid by the housing provider after a successful move-in and does not increase the cost of care or housing to the client or family. Not all providers participate in referral compensation arrangements. No Cost to Families - In most cases, our services are provided at no cost to the client or their family because compensation is paid by participating housing providers. If a situation arises where a fee would apply directly to a client or family, this will be clearly disclosed and agreed upon in writing prior to services being provided. Client Choice and Independence - Clients and families are under no obligation to select any housing provider recommended by Pathways Senior Housing Consulting. Clients are free to: • Choose any provider they prefer • Tour communities independently • Seek additional professional advice • Decline any recommendation. Our recommendations are based on the information provided by the client, available community options, and our understanding of each provider’s services. Provider Licensing and Regulation - Senior housing providers may include: • Adult Family Homes • Assisted Living Communities • Independent Living Communities• Memory Care Communities• Skilled Nursing Facilities. These providers are licensed and regulated by the Washington State Department of Social and Health Services (DSHS) or other applicable regulatory agencies. Pathways Senior Housing Consulting does not own, operate, or control any of the communities to which we may refer clients. Limitation of Responsibility - While Pathways Senior Housing Consulting strives to provide accurate and helpful information, we cannot guarantee: • Availability of housing at any specific location • Admission approval by a provider • Quality of services delivered by a housing provider • Future changes in provider policies or care costs. Families are encouraged to conduct their own evaluation of any housing provider prior to making a placement decision. Consent and Acknowledgment - By using the services of Pathways Senior Housing Consulting, the client or their authorized representative acknowledges that they have read and understand this disclosure regarding services and compensation.
